Important. Please make sure you fill out all 3 forms. One will folow the other:
-Booking Form: personal details necessary for your registration (note: scanned passport copy needed)
-Heath Form: information on your health and diet that need to be considered by the ship’s crew
-Ships Choice Form: your top 3 of preferred ships to sail on and if there is another trainee you want to sail together with.
